1. Who we are
Pathfinder Therapy CIC is a Community Interest Company registered in England and Wales. Company Number: 17248842. Registered office: Ribble Court, 1 Mead Way, Padiham, Burnley, BB12 7NG.
2. What information we collect
When you use our contact or referral enquiry form, we may collect your name, email address, telephone number, enquiry type, preferred contact method and the message you choose to provide.
Please do not include detailed clinical history, medication information, safeguarding disclosures or urgent risk information in the website form. If further information is needed, we will request it through a more appropriate process.
3. Why we use your information
We use the information you provide to respond to enquiries, consider referral requests, manage partnership or funding enquiries, and keep appropriate records of contact with Pathfinder.
4. Lawful basis
For general website enquiries, we may rely on consent and/or legitimate interests, depending on the nature of the enquiry. If an enquiry includes information relating to health or wellbeing, this may be special category data and will be handled with additional care.
5. Who we share information with
We do not sell personal information. We may share information where necessary with appropriate professionals, service providers, regulators, insurers, legal advisers, safeguarding bodies or emergency services where required by law, safety, safeguarding or professional obligations.
6. Website form processors
The website form uses Cloudflare for hosting, security and bot protection, and Resend to send enquiry emails to Pathfinder. These providers process technical and form-related data for the purpose of delivering the enquiry.
7. Cookies and analytics
Our website uses essential cookies and security tools to operate safely. With your consent, we also use Google Analytics to understand website use and improve our services. You can manage analytics cookie choices using the cookie banner or the Manage Cookies link in the footer. See our Cookie Notice.
8. How long we keep information
General website enquiries will be retained only for as long as reasonably necessary to respond and manage the enquiry. If an enquiry becomes part of a clinical, referral, safeguarding, contractual or governance process, different retention periods may apply.
9. Your rights
You may have rights to access, correct, erase, restrict or object to the use of your personal information, and to complain to the Information Commissioner’s Office if you are unhappy with how your data is handled.
11. Adaptive Immersive Trauma Therapy Pilot enquiries
Where you submit an expression of interest or professional partnership enquiry relating to the Adaptive Immersive Trauma Therapy Pilot, we may collect the minimal contact and preference information you provide on the public form. Please do not include detailed trauma narratives, combat details, full medical history, medication lists, amputation photographs, medical reports or identification documents in website forms.
If you later enter assessment or participation, special-category health information may be collected through secure clinical processes (not public website forms). This may include clinical assessment information, technology-platform session data relevant to delivery, remote-delivery arrangements, clinical outcome measures and pseudonymised service-evaluation data.
With your consent, we may share relevant information with your rehabilitation or healthcare professionals. We may also receive professional referrals. Information is shared only where appropriate and lawful, except where disclosure is required by law or immediate safety obligations.
Lawful bases may include consent and/or legitimate interests for initial enquiries, and additional conditions for special-category data where clinical care or service evaluation applies. You may withdraw consent for contact where processing is consent-based, without affecting the lawfulness of processing before withdrawal.
Technology platforms used for immersive delivery may act as processors or subprocessors. Where international transfers occur, appropriate safeguards will be applied. Data-processing agreements will be used where required. Retention periods for clinical and service-evaluation records may differ from general website enquiry retention. Deletion and access requests can be made using the contact details in this notice. Data-breach procedures follow Pathfinder’s organisational governance and legal duties.
Detailed trauma, pain or medical form responses are not sent to ordinary analytics or marketing platforms. Website analytics events for the pilot are privacy-conscious and do not include names, contact details, veteran status, therapy status, pain status, amputation details, accessibility details or free-text responses.
